To My Wonderful Patients and Their Families,
Due to ongoing staffing issues, it has become apparent that I am no longer able to care for your children in a way that I believe is best for your children and families. I am writing to inform you that First Choice Pediatrics will be closing. I plan to see patients at my office through March 7, 2025. I apologize for the short notice as this decision was unexpected and unavoidable.
Appointments through February 13, 2025 will not change at this time. Appointments between February 18 and March 7, 2025 will be for same day appointments only for a limited number of slots. After that date the office will remain open until May 15, 2025 with limited hours to help you transition to a new provider, but I will not be able to provide any patient care.
Your immunization records are available on the patient portal along with visit summaries. Please make sure that your portal access is up to date so that you may download this information before the office is closed. Your patient records can be obtained by providing a records release to our office via fax or mail. The fax number is 405-509-7778. Please note that there will be a fee to make copies of your chart.
If you have an outstanding balance on your account, I request that you please pay it promptly through the patient portal while it is still available.
I plan to continue practicing but have not found a new location. Future information will be sent via email messaging. It is vital that you have a portal account that is active. Ashley Hebert will be joining Hilltop Clinic which is located in Oklahoma City. That office number is 405-724-7482.
It has been my dream realized to “Help families grow healthy kids” at First Choice Pediatrics. I have enjoyed the last twenty-eight years of caring for your children, including a second generation of families. I am very honored that you have trusted me with your children’s healthcare. Watching your children grow has been a rich blessing to me.
